There is a high prevalence of depression among the general elderly population. There is an even higher prevalence among elderly receiving home health care, with a conservative estimate of 13.5% having clinically defined major depression. The Medicare Home Health Benefit provides limited coverage, eligibility, and payment for mental health services, and the national Outcome Assessment and Information Set (OASIS) assessment instrument has had limited requirements on assessing, diagnosing, and treating depression. However, the new OASIS-C significantly increases the depression-related requirements, though still not requiring assessments and not adding prospective payment reimbursement for patients with depression. The new OASIS-C requirements provide a gateway to home care providers seeking to expand depression care, especially combined with evidence-based models of primary care—based collaborative depression management programs such as the Improving Mood-Promoting Access to Collaborative Treatment (IMPACT) program. The article explains IMPACT, explores its cost savings and care improvement outcomes, and explores how Medicare and private duty home care providers might use IMPACT-type models to improve their patient care and agency fiscal health.

Hospices have been implementing various approaches to cope with the increased regulatory scrutiny in the past 3 years, prompted by concern about costs and outcomes. Medicare, through its fiscal intermediaries, has issued increasingly more restrictive local coverage determination guidelines for initial and ongoing qualifying criteria. The article examines how one hospice, United Hospice of Rockland, Inc. (UHR, New City, NY), designed multifaceted program to simultaneously improve regulatory compliance and improve quality of life. This article presents how the six scales (i.e., pain-verbal and non-verbal, breathing, functional decline-PPS and Fast, and anxiety) were developed and implemented, and also presents the preliminary evaluation of successful results.

In May 2005, Medicare proposed its first comprehensive revisions of the hospice conditions of participations (CoPs) in the then-23-year history of the Hospice Medicare Benefit. After 31/2 years of industry comments and government revisions, the final revised CoPs became effective December 2, 2008. This long journey created uncertainty for many hospices’ planning process and operations. This article recounts the experience of one mid-sized hospice’s now nearly 2-year planning and implementation process and the lessons learned.

A new concept is revolutionizing the way companies and consumers communicate in the digital age. That concept is social media—online communication that is interactive, collaborative, and democratic. Things that you will learn from this article are the following: What is social media and what does it have to offer the health care community? More important, how can home health care agencies incorporate social media into their marketing plans to speak more directly to their clients?

Introduction: The epidemiology of multidrug-resistant organisms is changing as evidenced by a shift to the community. Hospitalized patients are admitted to home health care with multidrug-resistant organisms. Numerous states have reported an increased incidence of community-acquired infections caused by multidrug-resistant organisms where reportedly no exposure to antibiotics or health care in the 3 months before symptom onset occurred. Method: A cohort research design with four separate home care agencies and a total of 127 nurses’ bags were cultured. Conclusions: The present study hypothesizes that the implementation of infection prevention and control strategies including education and training on proper hand hygiene and appropriate decontamination of high-touch surfaces (nurses’ bags) will interrupt the chain of transmission. Hand hygiene practices and environmental disinfection methods differ for Clostridium difficile than for other multidrug-resistant organisms, underscoring the need for addressing environmental factors. If present, implications for routine care and cleaning of nurses’ bags and specific changes in hand hygiene practices before and after home care visits are necessary.

The inclusion of "process measures" in the OASIS-C data set is a deviation from what home care providers have viewed as the "norm." To some, these new questions seem out of place. This article seeks to clarify the reasons for the inclusion of process measures and some of the concerns surrounding interpretation of these new data items.

Hospice home care and home health nurses are essential providers in efforts to improve the care and quality of life with families who have someone living with advanced or life-limiting conditions. Because these nursing professionals are part of end-of-life care, their duties include education about and disposal of unused medications. But drug disposal has broader implications, especially for the environment, aquatic life, and ultimately for human health. The purpose of this article is to (a) introduce the topic of drug disposal within an environmental and health context, (b) identify current recommendations for drug disposal, and (c) provide suggestions for hospice home care and home health care nurses for safer drug disposal.

Germs cannot be avoided. They live everywhere. Most germs do not harm people. The 2009 H1N1 influenza virus brought the concept of germs as infectious agents to the public’s attention, which generated fear of germs in some people. Home health nurses can reduce clients’ fear of germs by educating them about the differences between bacteria and viruses, two of the more common types of germs or infectious agents, and can teach them how to reduce their risk of getting diseases caused by these germs.
